About this opportunity
En tant que Médecin coordonnateur, votre mission est d'établir et organiser l'optimisation de la prise en charge médicale et psychosociale d'un patient dans un parcours de soins. Vous coordonnez la réalisation d'une démarche pluridisciplinaire selon le projet de soins établi. Vous apportez votre expertise et votre appui aux équipes médicales et paramédicales.

- Expérience: 5 An(s) - Hôpital ou HAD
- Qualification: Cadre
- Compétences: Actualiser le dossier médical du patient
- Déterminer les besoins thérapeutiques et réaliser les soins médicaux
- Recenser les symptômes, les dysfonctionnements, cerner l'environnement de vie du patient et procéder à l'examen clinique
- Réaliser la prescription médicale, expliquer les modalités de traitement au patient et le conseiller sur l'hygiène de vie
